It's been a long time since I last had a "Enter Your Name" screen on any of my games.
2011 was probably the last time I bothered.
Back then, the "Enter Your Name" screen would pop up every time the game over screen did.
In the end, it was frustrating constantly having the stupid thing pop up, after every short quick death, so when I created my Monkey framework in 2013, I decided not to bother including it.

Yesterday, I put it back in!
The name-entry option sits on the main (Start|Options|Credits) menu, and.. that's all it currently does.
It may be a while before you see it, though. It requires a little more tweaking for it to be functional.
